The first time our high-speed fan came on was after a half-hour of idling/creeping/idling with A/C on, in the line to cross the border at Tecate. In 90(some) degree heat. So it takes some extreme conditions.

(We didn't dare turn off our engine since we were having starter problems, and we needed A/C to keep from getting heatstroke!)
