I had something similar, but perhaps more irregular last spring. On
my way back to Texas from Colorado, the van seemed to sputter at
cruising speed occasionally (but only in 4th gear) to the point that
it lost power and I had to downshift. I could drive in lower gears
without problem. I too thought it was the fuel filter, but finding
one on the road in Oklahoma was impossible. Instead, I got some fuel
injector cleaner and ran that through a tank of gas. I think the
problem arose once more after that.
Strange thing is, I haven't had the problem since I got home. The
only thing I can think of in retrospect was that I got some bad gas
(I did stop at a sketchy gas station outside Denver) and it took a
while for it to work its way through the system. That's pure
speculation, and maybe completely ridiculous, but its the best I
could come up with.
