Ah, but the dealer doesn't only sell the 9004 (USA type). Even though the USA version of ETKA does not show the H4 E-Code style in the listings, they're available in the USA - a call to my contact at VOA garnered this up-to-date info with current retail pricing from their database (keep in mind the Dealer can charge anything they want [and most do]):

Low Beams 255 941 105 E-Code Style Left $156.08 - 12 Available in TX Depot 255 941 105 A USA 9004 Style Left $122.50 - 27 Available in LA Depot (plus others) 255 941 106 E-Code Style Right $156.08 - 18 Available in TX Depot 255 941 106 A USA 9004 Style Right $144.03 - 12 Available in LA Depot (plus others)

A more knowledgeable parts counterperson at the Dealer would have known about the H4s. Some USA Dealers (most don't) have the European ETKA installed to look up stuff like the H4s. Now the trick is to have them have the gonads to make the order for you.

(NOTE: I have been since corrected on this - ETKA Version 7 now being used at the dealers does show the North American and European listings).

As far as the adjustors and corresponding mount pieces that use the nylon/plastic - my personal experience says buy and replace them all and you'll be happer for it in the long run.
