Yeah in my experience as well, high idle is most commonly a vac leak. I've seen a couple intake runners get good sized rust holes in them at the bases as well, how much is rust a factor in where a hole might be?

since the idle speed screw has little effect, trouble may be significant vacuum leak, perhaps at the throttle body/intake manifold interface.
