You may think this a joke but my neighbour had this very thing occur this past summer. He replaced the bearings and spacer then did not notice the hub and nut were worn to the point that when the nut was tight at it's 360 ft. lbs. it was not touching the hub ! So, even with the cotter pin in place the nut worked its way free and they got to ride on three wheels for a short distance all the while watching the fourth wheel and tire go rolling off in the ditch behind them. Sure made a mess. He did manage to get it all straightened out and back together properly with the use of some good used parts I offered him from that old Syncro sitting out in my yard.
