It appears that the fanciest tool one would need is a hexagonal socket. It certainly doesn't make sense that there is an insert already because it would seem that if the bolt was siezed in it, the whole insert would simply come out.

I can figure how I would drill it out if it is spinning. I guess I can grab it with the vise grips to hold it while drilling. Then what? Drill out the full 14mm bolt? Drill and tap to put a bolt in the bolt, lever on that bolt?
