Chris, although I assume there can be many causes or combinations of causes that will cause those symptoms they are absolutely identical to the problem I had that was caused by an injector that was providing way to much gas. O2 sensor saw the gas, leaned out the other three. This was complicated by a miss adjusted throttle switch, a small part of the problem.
