Your numbers are a bit jumbled, but it appears that you get both an idle and 2500 rpm test, no Dyno so you are not being tested for NOx.

You are really way out of range. Even without a catalyst, CO should be somewhere around .6% and HC should still be below 200 or so. The spec is 0-1.2% at the test port before the Cat. If the engine was running right, a good Cat would bring these numbers down to "0" at the 2500 rpm test. So first you need the get the basic adjustments right. The fact that your AFM was tampered with may be part of the problem. With these readings, the Cat is most likely toasted.

I am at work now so I do not have much time. Later I will right up a procedure to help you get things right.
