POR15 should really be applied directly to unfinished metal. It isn't necessary to paint over POR15, but if you do wish to apply primer or paint, it should be applied over - not under - the POR15. (If using primer, apply it while the POR15 is still a bit tacky.) There is no need to completely remove the rust before you apply the POR15, but you do need to remove any loose or flaking rust and clean/prep the surface before applying it.

POR15 lasts longest in storage if you avoid exposing it to air and humidity for extended periods of time. Also, if you get rust particles into the can (say from a paint brush that you are using), it will do its job and react with the rust that's now in the can. So if you buy a large size, don't leave it open and dip your brush right into the can. Instead, pour a small portion into a separate container for your use that day, and then reseal the can.

I was a POR15 customer for years before I started selling it; I decided to carry it based on personal experience. I applied it to a Fiat convertible (a vehicle known for rusting, to put it kindly) and on a VW Bus. I sold both vehicles a few years later so I can't say for sure that the rust never returned, but at that time (several years later) it had not. That's quite a feat, especially here on the east coast (and on a Fiat, no less!). Of course there is no substitute for completely removing every speck of rust by cutting out those sections completely and welding in entirely new metal. But short of that major surgery, I know of no better rust repair solution than POR15. It's also useful for treating small rust bubbles before they turn into big problems.
