That picture doesn't look doctored to me. A long lens was used and focused on the boy. Since the bus was quite a ways behind him it's out of focus, but long lenses compress depth, so it appears like it's looming right behind him. Lighting looks fine to me, too. The sun is behind the photographer's right shoulder, at about the 2:00 position judging by the shadow of the boy's nose and elbow. The shadows on the left edge of the van, in front of the windwing, show the light was falling on the van from the same angle.
