If you're near a largish city, you can probably find what you need at a local shop. There is a (non-chain) shop in Tulsa that is almost 100% tires and wheels and they have been able to help with all my oddball lug nut and bolt needs - from putting aftermarket alloys on a Bug pan (under a kit car), to getting the right nuts to put wide 15" alloys on a Geo Metro that came with skinny 13" steel wheels. They very much predate the idea of 22" rims and fart cans on Civics - on their show floor, they've got gangster whitewalls and a copy of the Coker Tire catalog.

So far I have gotten the best results by bringing them at least the wheel so they can see what the nut seat is like. Bringing the whole car, or at least the parts that carry the wheel studs, is better. When I can't bring the whole car, I will first figure out the threads on the studs at home, using a gauge or regular nuts - then they can get me a nut with the right seat and right threads.
