Back in the day - George Washington set the precedent. He refused to
accept a title, or anything with his name in it. Ever the Virgina
gentleman, he said that simply "Mr. President" will do. I think that has
been protocol ever since. That way, the position of the representative
of the people - is elevated above the man.
