Remove all vinyl and fabric and replace with new. Replace entire ventilation system. Don't follow a smoker closer than 50 feet.

my moms '00 toyota avalaon was a returned lease purchase and the guy who orig leased it new was a smoker.. we tried that volcanic rock stuff in a mesh bag under the seat, and some citrus air freshner.. but the best thing you can do is just detail the hell out of the car.. clean everything as good as you can.. removing the yellow nicotine film will get most of the smell out..
