If a valve is smashed like this the valve adjustment will appear totally out of adjustment. It does not mean it is caused by a bad valve adjustment in the first place.

It's more likely a bad lifter losing its pressure causing the valve tip to be constantly bang'ed by the rocker then smashed like that. My 84 1.9 had a bad lifter when it was as young as 70k miles. It made a slight valve noise constantly. the valve tip was slightly smashed like the pictured one. But after taking the lifter out for repacking it was back to the engine. The 84 has since added 80k miles without any problem.
