That 3 groove pulley requires lots of torque (258 ft lb) whereas the single groove pulley only takes 43 ft lb. Is it possible that someone at some time had that pulley off and didn't torque it back correctly? If so, It's probably worn to the point of being useless now (along with the key). The worst case scenario would be if the keyway on the crankshaft has gotten beaten out of shape.

I hope it's simpler than that.
