The air flow in back of the fridge starts at the grill at the bottom front of the fridge. From there, the cool air goes underneath and then rises up the back when it gets heated. Then the hot air escapes thru the vents behind the stove. The side vent also exhausts hot air. Works like a chimney (convection). So, to aim the fan down would work against the convection current. I suggest aiming the fan up.

As far as putting a fan in the city water port, that will help exhaust some of the hot air, but for me, it was not worth it due to dust and water intrusion. The flap outside does not seal completely, so if you choose to put a fan there, you may want to redesign the flap, too.
