If it doesn't get hot at the boiler, it's guaranteed not to work. It's got a fire, a 12v/7.5A heater, and an equivalent 85-watt 110v heater to make it get hot. So if it's not getting hot, fix that, and then worry about whether it gets cold on the other end.

And no, you don't need both voltages to run it on the bench. If you apply twelve volts in the right places it should run continuously -- on 110v it's controlled by the thermostat.
