Or the pilot bearing could be seized/galled.
Another possibility is that the disc fell apart and all the pieces are stuck
in there.

Hi Luke,
Get a helper to work the pedal while you watch the action at the
transmission. Also; check under the bellows on the slave cylinder; my
leak was contained there until I poked at it with a finger and it
suddenly rained down with fluid.
It certainly could be a problem inside the bell housing, in which case
the transmission will need to be removed. If the slave cylinder is
moving the lever fully, you will need to drop the transmission. If not,
the problem is with the hydraulics2; either at the slave cylinder or at
the master.(or in the line that connects them).
