I have no idea if this is the "Recommended" way but here's what I did successfully.

Once the heads were removed, I "Double nutted" the tops of the studs and used a mapp gas torch on the case where the studs screw in then turned 'em out with a wrench on the lower nut of the double nut arrangement.
