Neil, I have a small Pittman arm puller I use for the lever. There is very little for it to grab so if the lever is stuck the puller will spread since the puller jaws are against an angled surface. I then arc weld a couple bumps for the 2 jaws to catch against and that usually does it. I have had to cut a few off with a sawsall still. Then I use a press to force the cutoff stub out of the lever.
