Hi Brady, That happened to me more than once years ago. I found that the hex bolts can't be torqued down as much as the 12 points. Hasn't happened since going to all 12 pt cv bolts.   Robert 1982 Westfalia

So I'm in Boston in traffic. I just missed seeing a Volvo hit a guy on a bicycle. Traffic is a mess, ambulance, fire and police are trying to get through to the seen. I'm pissed cause the Volvo and bicycle are screwing up my time line ( yes I'm not being nice) radio is blasting. I finally get through the intersection with all of this mess happening. I went to the next light and when it went green I took off....BLAM well I knew what that noise was. Pulled over out of the way of traffic so as to not screw up every ones commute take a looks at the axle and... Well 3 of the 6 CV bolts ain't enough to Handel a quick start with BFG 31" Well I just locked the rear diff and drove another 40 something miles. I love my Syncr-Ho!
