Sadly I can think of only two ways for oil to get into the coolant -- head gasket leak, or oil cooler. I'm not familiar with the Vanagon oil cooler so I don't know whether that route is a possibility.

OK, there is a third possibility I guess: the kid checked the oil, saw it low, and dumped oil into the overflow tank instead of the oil filler. They are right next to each other after all.
