Except that your actual head would block the view. Besides, your rear view
mirror generally points toward the rear hatch, which is straight between the
seats, not through the center holes in the headrests.

This is silly. Try it youself. Sit in your drivers seat and see how much of
the outside of your van you are actually viewing from the hole in your
headrest.  If you think you are looking through the headrest (and your head)
to see your rear window, then you should keep it clear of all objects
(including, for that matter, your head :-).

As BenT suggested, the style back in the 80's was for "holey" headrests. The
style in the 2000's is for solid ones, so that's what most modern cars have.
Physics (and human physiology) haven't changed in the interim, but headrests
have.
