There are a number of other pieces that can break including the operating shaft in the bell housing and the pin on the clutch pedal.

Vanagons have hydraulic clutch mechanisms all the way. No cables. It's almost got to be a clutch master or clutch slave cylinder failure. Clutch master under the dash, clutch slave mounts on the side of the MT. Dunno where it is on the aut tranny version but the same I suspect.
