May or may not be your problem, but worth giving a look. Friend of mine had exactly the same mysterious problem, and he and I spent all day on it, and swapped every component, desperately trying to fix it. Ran out of time, so had to give up temporarily. He brought it to a mechanic, who promptly snipped the shielded cable for the O2 sensor at the ECU end, and added a new wire to the sensor. Good as new, problem solved! Somewhere the cable was shorting internally, and after the van came out of warm-up, went full rich with the fuel mixture.
