too: Replacing a Lost Title
To replace a lost title, complete Form TC-123, "Duplicate Title
Application"<http://tax.utah.gov/forms/current/tc-123.pdf>.
This form can be obtained at any DMV office. The Duplicate Title Application
must be signed by an owner of record and may require a release of lien from
previous lien holder. The fee for a duplicate title is $6.00.
