In immediate context this means he's having some difficulty with the
catheter in his abdomen that he uses to inject dialyzing fluid.  The
planned departure for Maine is being put off while they fiddle with that.

The longer-term bad news is that his cryptococcus infection and vast
numbers of transfusions have compromised him severely as a candidate
for a kidney transplant.  This one will have to play out over the
next two-three years, but it's a severe blow.
