'Catastrophic failure' means it blew itself to smithereens... or something of that order.

you're having a running issue. Hardly catastrophic. Funny how people love to dramatize it , evidently.

the idea of clogged cat is good thinking. that's a drastic difference between the lowest cylinder and the highest.. 95 versus 165.

that fuel pressure is way off. start there. vacuum leaks seldom make much noise. would be good to have a spare known good MAP sensor to try.

you have to check your fuel filter once in a while. Surprised anyone would keep driving their van knowing fuel pressure is half of what it should be, unless the requirements for Haltech are different than stock.

I won't post the definition of 'catastrophe' . Maybe it's an 'officially weird day.'
