That's Stuart's rig. He's on the Syncro 16 List and other Syncro forums. I don't have my records here but that 16" is one of maybe 2-3 existing in the whole continent of Africa. I know Stuart was in the process of restoring the Syncro 16 which was used by the Soviet diplomatic mission in Cuba and subsequently in South Africa. Maybe this is the same van.

Yes, there is such a thing as an OEM rear spare tire carrier. IIRC, they sold for about $1200 when you could order them. That's because you had to order them in pieces unless you ordered one when you bought your Syncro 16 new.

The SA steering wheel looking wheels came on Eurovans or T4's as they are commonly called elsewhere.
