Possibly a bad AFM connection. Standing behind vehicle, it's at the far right corner (passenger side), attached to the air filter. Has a connection toward the front of the van. Check that it is firmly connected. Wouldn't hurt to remove it and coat the contacts with dielectric grease. Could be a host of other problems, but wiggling and tightening all FI connections is a good start.
