I had symptoms JUST like that for a LONG time.  Took an age (a year) and
many trips to a variety of mechanics and many cases of sudden engine death
and resuscitation on the road before it was finally bad enough that my
mechanic saw the problem and could diagnose it - the AFM.  Once a got a
replacement, and once my mechanic accepted that he did have to adjust the
settings on it, the problem stopped.

But apparently it could have been a variety of other things as well, so
yours could probably be something else.
