Mike Wing should be able to spot camber and caster problems straight away, although why the thing would have slipped so badly would need to be discovered. As for Idea #3 ("[...] hardware problems in the suspension that make everything look right on the alignment rack but don't work right on the road.", this sounds like something for my go-to mechanic (Steve, at Steve's Place here in Bend) to visually inspect for.
