I use the difficult both feet & 2 hands technique to hold wheel till I get bolts in place. Place butt on ground in front of said axle, lift up wheel w/ hands & hold steady w/ sides of feet while aligning wheel w/ arms/inserting bolts.

I think even w/ a fused ankle(?) one could do this, but I might be wrong again.
