I have enlarged the hole to accommodate 5.25" speakers. I think removing the panel and using a jigsaw would be easier/cleaner/safer than trying to rig something to center the hole saw on the existing hole and cutting with the door panel installed. Beware of the protruding metal behind the door panel.
