Bad adapter.

Buy quality stuff, cheap stuff will cost you more.

I good friend runs a GoPro Hero on his windshield, he powered it with an
aftermarket USB adapter.  One day the unit would not power off, he ended up
having to unplug it.  Checked the USB adapter and it was putting out full
voltage to the Hero.  The Hero is fried!  Now he only uses the GoPro
adapter.
