Based on your description the hose you need is 25mm (1") at the end that plugs onto the head and narrows down to 16mm (5/8") for the heater junction tee. There is a common VW hose that does this, under $10

It will be too short. The Vanagon hose normally goes bad at the head end. You can cut off the bad end and splice in the above hose that is about a foot long. Use a 5/8" hose splicer to attach it to the remaining original hose. Or get some additional 5/8" heater hose and splice the 2 new sections to make a new hose the full length.
