Jack, if that's a list reply or similar with a Re: or Fwd: at the begtinning of the Subject: field it means that someone else's ISP or email software thought the message was spam and inserted that into the subject to tell them so. They saw it was a list message and replied to it without remembering to take [SPAM] out of the subject header. If it's a message direct to you then it's probably your ISP informing you that the message fits their test criteria as suspected spam. If you're using web-based email it would be the email provider and not the ISP. If you have a web-based provider but retrieve your mail using conventional email software it could be both.
