On vehicles that use full sized spares (Vanagon for example), I do five tire rotations, and replace all five when needed, so the condition you fear never materializes. When I had the storage space for it, the first thing I would do when I bought a new car (I don't do that often, mind you) would be to buy a wheel and a full sized spare to replace the donut. But, with tire quality being what it is now, and without adequate storage room, I haven't done that for the Prius - still have the donut.
