My guess is that even if the AFM flap didn't get pulled open when cranking engine (it likely does though), the air bypass (CO adjuster screw) would allow little air in. Details of the AFM being used as in input to ECU when starting engine, might be mentioned in the Digifant section of the Bentley? I know that section describes functions of various inputs like the coolant temp sensor.

Zoltan: what engine? Coolant temp sensor and wiring to/from ok?

My thought regarding the fuel pressure residual test was dripping injectors at shut off. Check the plugs once the hot start issue shows up? See if they're wet with fuel?
