If you're saying that your filter is set up with both of those things as parameters in the same field, then I'd guess that the emails that are slipping the net are missing one or the other (my guess would be the <mailto:etc ... > ; one of them might be getting stripped out on the way 'round. Hard to tell without looking. The filters are quite literal in their execution. Compare the ones that didn't get filtered and see if anything at all is missing.
