Pull the upper top and back channel out, pull the inner and outer scrapers off( the area where your arm rests) there are clips inside the door which hold  the scraper, at the front next to vent pull the 2 nearest the vent out as you will need the room. At the top of the door above the vent is a small Phillips sheet metal screw, remove it. 
    Pull the door panel and locate the 2 bolts that hold the door glass to the regulator and remove them. There is a sweet spot in the door where the door glass will lift out. It involves lifting the front and tilting the rear. Take the door glass out ( You can not take the vent out with the door glass in place) .
      There is a single 10mm bolt at the face of the door which holds the lower part of the vent, remove that, grab a hold of the assembly and tilt it backwards and manipulate it out of the door. I should have asked if this is an early van with Chrome in the doors, if so you have to remove the rubber lip which locks  into the trim before you remove.
        As the English service manuals say " install is the reverse of the above procedure.
