I use a 6 point socket that fits the lug nuts of the car, it's
attached to a breaker bar and each vehicle I have has one with it for
changing the tires. On more than one occasion I have gently bounced on
the end of the bar and eventually loosened the nut.

On the side of the road, I estimate as best I can with body weight and lever length, and then get it properly re-torqued once in town.
