I'm working with Steve Schwenk to get a web form going for the head
gasket survey.  I'll leave it to Steve to give the green light when
we're ready, but the web form is on my page at:

I only tested it with MSIE, but it seems to work OK with that.  I'm
out this weekend, so if the survey changes, I can't update the page
until early next week.
