No, the hub can stay on while doing the brake job, just remove the drum.

Depends on your paranoia, really. The dust excluder boot's ("rubber
grommet") job is to keep the brake dust out and from contaminating the
cylinder. If it's not leaking, it may be just fine, but it's purely up to
you. Price for replacement cylinder is $31.50 each for ATE/FAG German,
$12.95 each for Varga Brazil (I run Brazil on mine - haven't had any
problems over the past year and 23K miles).

VW part # N 102276 - Size is a 7 by 12mm length, kind of a bastard size.
VW gets 52 cents each for these. I would recommend re-installing them.

I don't feel the cable is the problem, just the rust within the brake/drum
area. But if there's a question of them working properly, by all means
replace them. I'm sure Ron carries them and I usually do as well. My Part
Number 251-6261C - fits either left and right on ALL 2wd 80-91 and I get
$11.95 per side
I had the same problem crop up this winter on my 90 Fox Wagon. Replaced
the shoes and thoroughly cleaned the brakes area and the drum (was a little
rusty and I use the car daily).
