Depends on your paranoia, really. The dust excluder boot's ("rubber grommet") job is to keep the brake dust out and from contaminating the cylinder. If it's not leaking, it may be just fine, but it's purely up to you. Price for replacement cylinder is $31.50 each for ATE/FAG German, $12.95 each for Varga Brazil (I run Brazil on mine - haven't had any problems over the past year and 23K miles).

I had the same problem crop up this winter on my 90 Fox Wagon. Replaced the shoes and thoroughly cleaned the brakes area and the drum (was a little rusty and I use the car daily).
