Fourth gear is probably broken at the hub. Further damage will include the main bearing and possibly some of the housings. The 3-4 slider hub will also have to be replaced. Depending on who is doing the work, and how well you wan the Trans repaired, figure $1,500 to $2,500. Especially with a clutch replacement, Trans mounts, slave cylinder, etc.
