James,

Was the tube that you replaced on the left side of the engine? If so, you probably didn't get one of the pushrods centered properly in the lifter. If the pushrod gets hung up on the shoulder of the lifter, you will experience your problem. It is easy to have this problem when installing the pushrod horizontally, as when you do it with the engine in the van.

I also assume that you had cyl #4 at TDC when you set the valve lash.
