What happens is that the throttle shaft wears over time causing the throttle plate to sometimes not close all the way--thus the throttle switch stays open at idle. The fix is to open up the throttle switch gap just enough so that the switch always closes properly at idle. For my throttle body, a gap of 0.178mm did the trick. If you get the gap too large, gas mileage will suffer noticeably.
