Chris -- It sounds like you are having all the nasty standard vanagon problems known to the list, but all at once rather than one or two per year. Ugggh.

From the sound of the tranny, it's the standard 3/4 problem which Todd Hill describes here from time to time. I'm sure he'll use better terminology than I can, and be more accurate, but anyway, yep, it's broke, and I doubt there's nothing you can do but take it in to get rebuilt. Ouch. But hey, Todd is in California too...

The starting problem is likely another infamous vanagon problem, known as the 'sticky solenoid'. Lots of folks (and I must admit to having been one of them for a while myself...) deal with this using a hammer: when the car won't start, put it in gear, put on the emergency brake (well, if you have one!), make sure that thing can't move, and slide under the side in front of the passenger side rear wheel, and give that solenoid a few gentle taps. It'll start, most times. After a while that starts working less often, at which point you disassemble and clean your solenoid, get a new solenoid, or get a new starter. I did the latter as I was in Boulder CO and home is in Utah.

The emergency brake problem is most likely minimal. I'd guess you wore the hell out of your shoes, and now the emergency brake lever can't pull them to the drum. Probably need to pop the little covers on the back of the drums and turn the adjustment wheel by hand until you feel the brakes start to engage, then turn it back until it's free again. It's a pain working in there, but not too bad other than that. As I recall, there's a direction that it's pretty easy to turn the adjustment wheel in... that's the direction that moves the shoes closer to the drums. You might open up the other little cover back there and see if there's any shoe left at all ;)

Ok, there's my take. Except for the tranny, it sounds like your problems aren't too serious. The tranny sucks though, and will probalby require you to find a used one or get the trashed one rebuilt.
