Well, actually, the valve guides wear out around 200,000 miles, and you can expect to replace the lifters at least once, and you can expect to resurface the exhaust manifold a couple of times (if it's not a turbo), etc. This is from experience.

Were I spending the big bucks on installing a 5 cylinder in my vanagon, I'd go through the engine, regardless of the milage, if for no other reason than to replace the gaskets and seals. Granted, I think I could get ahold of a complete used engine, and rebuild it for a lot less than $4500, but still it could run into some money.
